WindJack Solutions, Inc has released AcroDialogs, a graphical designer for creating Acrobat JavaScript Dialogs. This Acrobat plug-in makes creating your own custom dialogs quick and easy.    
When Adobe released Acrobat 7 they documented the most useful User Interface device they have ever created for Acrobat JavaScript- fully featured dialog boxes. However, providing this capability was a bit like dangling a carrot since dialogs are complex code structures, and there is no easy way to create them. We know from experience how frustrating it can be to write code for these dialogs, but we also know how useful and powerful they are to incorporate into PDF documents and forms. That’s why we created AcroDialogs.
All you do is drag and drop Buttons, Edit Boxes, Images, etc. onto a dialog design. Rearrange, resize, and copy/paste dialog box elements. If desired add in extra bits of code for extending features. Create dialog wizards complete with Next, Previous, and Finish buttons. Test dialogs any time from within the AcroDialogs tool. AcroDialogs generates well-formatted JavaScript code. This code can be used anywhere in Acrobat or PDF where JavaScript can be used, including LiveCycle Designer, and FDF files.
You use dialogs all the time in software applications. In a PDF file they can be used to create a document splash screen, customized alert boxes (with your own images and special buttons), and sophisticated input panels. Use to collect or select special form data users may not have at hand (part numbers, department codes, etc.). Use dialogs at the folder level to create plug-in like control panels for automating Acrobat. Use in an FDF file to give users a professional looking response to their form submission.
