SOLVED: It worked when I added an actual on-page TOC too (as well as the navigational TOC), not threaded to the main story.
Hmmm. Slight progress in that I can get the space OK if I place the images inline rather than above the line, but on export I get this warning: ‘Custom Layout settings in Object Export Options have not been honored for group children, button states and inline objects.’ The epub looks OK, and it converts to Kindle, so should I worry about that warning?

I’m getting this validation error too (although everything looks OK in previewers and an actual Kindle). Here is the relevant bit of the generated CSS. I actually started with the ebook template made by Anne-Marie, so images had the object style Above line centred. Any ideas what’s causing this please? Should I just ignore it?
