Yes, linking from one document to another is always fraught with issues. I’ll do it, but not happily.

Wow, that’s very odd. Are you looking at the exported PDF in Adobe Acrobat or Reader? Or is it some non-Adobe PDF reader?
I would probably try to re-place the images. If that doesn’t work, I might rebuild preferences. If that doesn’t work, then I might export as IDML and open the IDML… Perhaps something got corrupted somewhere.

Oh… hm… I cannot think of any way to do that with InDesign. One numbering level cannot refer to some other section or list, other than an earlier Level. Might be possible with a custom script, if you had to do this often.
