David BlatnerKeymasterThis is really far better done in Photoshop than InDesign. But the word “cutout” is not clear to me. It may mean “clipping path,” which would indicate drawing a path in PS's paths panel and setting it as a clipping path there.

I'm not sure what you mean by the Kindle version being more expensive. Peachpit's kindle version is $3 less. (You could argue that it should be much less as there is no paper and inventory, but that's a different issue.)
Oh, I bet you're referring to Liz Castro's epub book being so much more than the current “sale” price for her book.
David BlatnerKeymasterInDesign itself cannot do this… I usually recommend eDocker for this kind of thing. It's surprisingly inexpensive compared to hiring a developer!

David BlatnerKeymasterYes, InDesign CS4 (and cs5) has a feature called Line Styles (right next to nested styles) that lets you assign a character style to the first line of a paragraph, or the second line, or whatever.
Nested styles work by specifying an “end character,” like “apply boldstyle up to the first 4 words” or “apply italicstyle up to an exclamation point.”

David BlatnerKeymasterI find File > Print to be very slow the first time I use it, but then it speeds up. My understanding is that it's just part of the “modern print architecture.” However, I wonder if you should use the Printer system prefs to delete and rebuild your printer.
David BlatnerKeymasterI agree; this is frustrating. I usually just press cmd/ctrl-2 or -4 to zoom in to 200% or 400%. But if the text cursor may not be visible at all if it's selecting something that is overset (outside a frame). Try pressing Cmd/Ctrl-Y to open the story editor. Even better, set the story editor preferences to Barbell cursor, so you can see it even easier there.

David BlatnerKeymasterKirk, you and I are using different terminology and talking about different things, I think.
My point is that if you set up your layer comps in Photoshop properly and then used the script, and then used Acrobat to merge optimize properly, you would get the same results as you did from InDesign. That's all.
But no, I don't think there is any better way to automate this.
