David BlatnerKeymasterI will say that I HATE making hyperlinks from one InDesign document to another. They are notoriously terrible and troublesome. One idea: make sure all the InDesign documents are open when you make the hyperlink and when you make the PDF. Or, better yet, if you can put all the InDesign documents into a single INDD file, you're likely to be happier in the long run.
David BlatnerKeymasterFirst of all: There was no major change between CS6 and CS5.5. This has always worked exactly the same way. If you are seeing a difference, then maybe something else is wrong.
The best way to make the first page a left-hand page is to select it, choose Layout > Numbering & Section Options, and then change it to page 2 (or any even number). If Allow Document Pages to Shuffle is on (in the Pages panel menu), then all the pages will shuffle and you will have three spreads, each with 2 pages.
The method of turning off allow pages to shuffle is very powerful, but much more difficult, and you only need it if you are using Automatic Page Numbering AND you need the left-hand page to be page 1 (which is very rare).

David BlatnerKeymasterInDesign has never had a way to show pages horizontally in the document window, but the Pages panel might do what you want:

Otherwise, you'd have to export the pages as images and view it elsewhere.
David BlatnerKeymasterBTW, after downloading that snippet file, you may need to change its name to remove the .txt file extension that sometimes gets added. It should be “.idms” — then you can use file > place to place it into your document. It's just a paragraph style that provides a rule, based on the images above.

David BlatnerKeymasterGreat question! This is not obvious: Choose Content Placement Options from the data merge panel menu and turn on the checkbox labeled Remove Blank Lines for Empty Fields. (This also shows up in the Options tab of the Create Merged Document dialog box.) I should write a blog post about that!

David BlatnerKeymasterI'm not sure I understand. Are you talking about footnote numbering, or making the footnote text go across pages?
Check the Type > Document Footnote Options dialog box for some help.
Also, search this site for “endnotes”. Unfortunately, InDesign does not do endnotes itself, but there are some scripts you can use.
David BlatnerKeymasterIf you're on the Mac, I strongly recommend the add-on utlity called Default Folder, which can get you a little preview of most images.
David BlatnerKeymasterI can't think of any clever solution immediately. Maybe something with conditional text? Or perhaps put the changed text in an opaque frame above the actual TOC, so you cannot see the “real” toc text, but can see the numbers?
