To see the scaling percentage of the image inside the frame, you need to select the image inside the frame. (It’s a different “object” than the frame itself.) Double-click the image to select inside the frame. (Press Esc to switch back to the frame itself.)

Yes, I believe that is a known bug in 2015.4 and Adobe intends to fix it. But I haven’t seen a fix yet. Frustrating.

If you get All Caps when you apply a paragraph or character style in InDesign, then perhaps the All Caps formatting is part of the style definition? Did you look at the style definition (by editing the style) and see if All Caps or Small Caps is turned on?
In general, you should different master pages. However, you’re right that sometimes you want the master page to change based on the content of the page… I believe AutoFlow can help with that: https://in-tools.com/products/plugins/autoflow-pro/
