If InDesign is going to have *any* trouble, it’d be with a Word doc with “lots of cross-references and indexes.” If it’s at all possible to write this in InDesign itself, that’d be preferable. Or, use the Word file as final for publication.
Otherwise I’d test the Word > ID import periodically to make sure all is well … check xrefs, mainly. Also, I’m not sure how you’re going to be able to do an index in Word that updates itself in InDesign with the new/correct page numbers. Did I cover that in the title? hahaha … I don’t remember.

Reflowable EPUB 3 ebooks don’t support multiple columns. You can get that with a Fixed Layout EPUB though.

I vaguely recall that, it may have been fixed in a CS6 patch. I’ll nose around.

Are these automatic numbered/bulleted lists? (And if so, did you leave them as such or did you turn on Convert to Static in the export to EPUB dialog box)
And what version of ID are you using?
I ask because possible reasons and fixes depend on how they were set up in ID and how ID does with the conversion, which is dependent on the version.
