Layout – headings, numbers vanish

      Where am I going wrong?
      My book is laid out as I want it in InDesign. Every detail has been double-checked. The headings etc. are all correctly located.
      Then I save it as a PDF ready to upload to Lightning (as I have done previously with other books without problems).
      In the PDF I see that page headings and page numbers have disappeared on many pages. And on other pages page headings and numbers have appeared twice, on top of other headings and numbers.
      No matter how many times I try to correct this in InDesign when I save the document to PDF the same errors appear.
      There must be a simple solution, right? Have I somehow messed up the Document Setup? All suggestions welcome!
