It’s fixed! I worked it out with a bit of trial and error. It’s amazing what sleeping on it can achieve. It’s hard to explain, but basically the style I had set for each level in the TOC dialogue box was based on the paragraph numbering styles used in the document, thus causing a some sort of compounding renumbering effect.
