I made a new document with a paragraph style for the headlines and a nested character style in it for the strike-through. If I understand your description, it seems to work very well. You can download the InDesign-file (in case you use an older version I saved it an IDML, too). You can download the files here:

MemberI select the whole text (“John L. Smith, MD,”) that I want to show up in my index, with the mouse. Then I open the index panel and click on the button to create an index entry. Here it shows as “John L. Smith, MD,”, which I change manually to “Smith, John L., MD,”. Finally I let InDesign create the index and voilà : it’s “”John L. Smith, MD,””.

MemberDo you really need ‘true’ spaces? Otherwise you can make it look like spaces with a character style that applies some character distance.

MemberFind-Change is not possible as part of a paragraph style. GREP within a paragraph style only works as embedded character style.

MemberYou have to use character styles instead before converting the footnotes to endnotes and vice versa, then the formatting will not be lost.

MemberFor me that sounds like: Bleed of 3 mm for top, 3 mm for bottom, 3 mm for outer edge (left side on even pages, right side on uneven pages), 0 mm for inner edge.
