In the footnote options, layout section, “Rule above:” you can choose between first footnote and continued footnote in column.

I select the whole text (“John L. Smith, MD,”) that I want to show up in my index, with the mouse. Then I open the index panel and click on the button to create an index entry. Here it shows as “John L. Smith, MD,”, which I change manually to “Smith, John L., MD,”. Finally I let InDesign create the index and voilà: it’s “”John L. Smith, MD,””.

Can you describe the paragraph and character style that you have applied? Your screenshot is too small.

Are you searching through the whole document or just in the story?

That is strange. Find-Replace with InDesign CC 14.01 gets me HellX WXrld.

For me that sounds like: Bleed of 3 mm for top, 3 mm for bottom, 3 mm for outer edge (left side on even pages, right side on uneven pages), 0 mm for inner edge.

When you say, you I tried to import the document into InDesign, do you mean copy/paste or place?
