David BlatnerKeymasterIt depends mostly on the program that created the EPS. The key is the listing inside the EPS file of the BoundingBox (usually listed near the top of the file; try opening it with a text editor to see). If that number changes, ID may resize the file on relinking.
If you don't want an image to resize when you relink it, turn off (in the File Handling pane of the Preferences dialog box) the Preserve Image Dimensions When Relinking checkbox.

But I absolutely cannot make InDesign give me that dialog box now — both in CS4 and CS5! How frustrating.

David BlatnerKeymasterI'm glad it's worked out for you, but… This is a real mystery… I've never seen that alert.
I wonder if perhaps you have some third-party plug-in installed that you might not realize is running? Are you using DataLinker or some other spreadsheet/database linking software?
David BlatnerKeymasterYeah, generally, you can just export IDML and open the IDML file and the swatches will be “fixed.”
If you still can't find the color, you might try the Edit > Find Color feature with Blatner Tools (blatnertools.com)… it can sometimes uncover where a colored object is, even when you think you've gotten rid of them all.

When you say “no to speeding up the process by unformatting it” what do you mean? I have never seen anything in InDesign that asks this.

I think the skills that are needed in the USA are the same as everywhere: understanding of the basic technologies and concepts, understanding of workflow and color management, understanding of scripting/automation.
David BlatnerKeymasterYou could just use nested styles, which would be easier. When making the nested style, click on the word “Words” (which gives you a popup menu of options, such as Sentences, Characters, etc.). Instead of choosing from the menu, type : – (colon, hyphen) or use em dashes or whatever. The character style will span “up to” or “through” (depending on what you choose) whichever character it comes to first.
That is, if you type .,! then it'll extend to the first period, comma, or exclamation point.

but you want to do it with no 1… wow, it seems like it should be simple, but I can't think of how you would do it! Maybe you could put a “1.” on an invisible paragraph. But you need something to increment the first level.

The other thing that has caused me no end of crashes is shared hyperlinks, especially those that are across multiple documents, or that use shared destinations from other documents.
David BlatnerKeymasterSo Preflight and Conditional text? Neither of those crash for me, nor should they crash for you. I have heard that preflight can slow down on a really big document. Hm.
Other than IDML and rebuilding preferences, I'm not sure what to suggest. Perhaps try this on another computer, or create a new log-in account on your computer and try it from there.
David BlatnerKeymasterIt's a great question and something that has bothered many people for years. For some reason the Command-H “Hide” shortcut is not available in the Keyboard system preferences pane, so you can't disable it there. The only way I have found to disable Command-H is to use a third-party utility such as iKey, which lets you remap your keyboard shortcuts.
Somehow iKey (and perhaps other utilities, such as QuicKeys?) captures the cmd-H before the mac OS does, so it seems to work.
