As far as I can tell, it’s a system or browser problem. I am often logged out, but I can’t figure out why. I notice that it happened MUCH more frequently since the last time I updated my Mac OS to 10.9… or 10.9.3 or something.
That is great that you are keeping previous versions. Documents can go kablooey, but as you said, fortunately it is very rare.
The best bet is just to figure out what might have been changed… for example, it could be a bad font, or a corrupt image that gets added to the file. Or it could be a glitch caused by working off a server. It’s probably impossible to track down without watching until it happens again.
I would just do a find/change like this:

But I think the key for you is: you’re probably going to have to upload those images into the CMS manually somehow, and then change the links to point to them. It’s not much fun.
I know of some people who are doing InDesign to WordPress links, but most of it is “custom solution” so it’s not really “off the shelf” for people to buy.
Actually, that article I linked to was written in 2011, so perhaps he used CS5! This should definitely work in CS6, too.
Tom: I’m curious why you would do that instead of using a Book panel, which handles all the page numbering for you? And the Index feature can handle the index for all the documents in a book panel without you having to “hand merge” them.
In the Index panel you should be able to turn on the Book checkbox in the upper right corner.
And in the Generate Index dialog box, you should be able to turn on the Include Book Documents checkbox.
