Yeah, you’d need to do that as a separate text frame.
As Amanda pointed out, you could anchor it into the text… but there’s a trick: the text wrap of anchored frames ONLY affects text AFTER the line of text where they’re anchored. So in this case, you’d need to anchor it in the paragraph starting with “Designer’s such as Stuart…”
It’s worth checking to see if these are “real” footnotes, or if someone created them manually.
For example, try inserting your own test footnote somewhere near the beginning. Do all the other numbers update?

That does seem very strange to me. Changing the margins can be an issue (as Steve pointed out), but JUST changing bleed and slug shouldn’t cause that kind of error.
It is possible that there’s some weirdness going on with the file, so perhaps try exporting document as IDML and then opening that IDML file and trying again.
I don’t think there is a way to turn off Adjust Layout in the Document Setup dialog box. That button you see isn’t actually an on/off switch — it’s a way to open the Adjust Layout dialog box. Basically, it’s a way to get to use Adjust Layout INSTEAD of document setup.
What is happening when you try to add a slug area? Maybe the slug area needs to be smaller? (When I try adding a slug area to a page that size, it says it has to be smaller than 36p)
The problem is that those titles are no different than any other text in your document. GREP can only work when there is a clear pattern that makes text different. For example, if the book titles had quotation marks around them, then you could say “make all the text with quotation marks around it” italic. Like this:
