Running head variables don’t pick up character styling (like italic text). I believe the best way to do that is with Power Headers from in-tools.com
This may be a problem based on what PDF reader they’re using on the phone/tablet. Some pdf readers support forms, and some don’t. Are they using Adobe Reader?
There is a LOT that mobile pdf readers often don’t display properly. Frustrating.
I believe it’s possible, but it’s not fun. Instead, you might take a look at using

or, probably much easier:

It sounds as though you want the same InDesign document to work for both print and epub. That can work, but you often need to create a custom CSS file that you attach to the epub when exporting. Often, people just take their print indesign file and then make significant changes to it to make it epub-ready (so they end up with two different INDD files).
Sounds like you want the Cross References panel! :-)

and if it’s going to a specific piece of text, you might want to cross-reference to a text anchor:

John, wow! You found a “bug”! I don’t recall anyone ever reporting that one before.
I’m glad you’re enjoying the book, and I apologize for that mistake. I honestly don’t know why we would have referenced that. You can find a section on Setting Defaults in chapter 1, I think, where we explain how you can change the defaults for certain features by changing them while no documents are open.
You can learn more about his here

Does it show up in any other programs on your computer? Does it work on other computers? Might be a font corruption/weirdness problem? Maybe remove it from Suitcase and then re-add it? Or find a clean copy to add it?
Yes, as Peter learned, code on our forum needs to be inside pre or code html tags. :-)
Thanks to Keith and Peter for jumping in and helping!
