The Title and many other fields in the PDF properties are controlled in InDesign by File > File Info.
I don’t know any way to automatically populate the File Info dialog box with the document name like that. (In most cases, you would NOT want the document title to be in the PDF metadata… but I can see why you do want it in this situation!)
David BlatnerKeymasterThat is a very good question about web vs. print logos. Personally, when I work with logos, I always have an RGB version and a CMYK version — even if they’re vector. That way, I can ensure the RGB is right and the CMYK is right.

The purple bar problem is extremely annoying. I haven’t seen it recently, though, so it might be dependent on which version of Acrobat you’re using.

I don’t know any way to do this for free. :-)
David BlatnerKeymasterHm. Did you know there is a “Print as Bitmap” feature in the Print dialog box? Will that work to print to your printer?
David BlatnerKeymasterI don’t know of anything specifically. On the Mac, you could probably create an applescript that exports an image from InDesign and then converts it to Photoshop Bitmap.
I’m curious why you want a black and white bitmap like this?
