David BlatnerKeymasterI think there is a relatively easy way to do this, by 1) making an index for each document; 2) doing a find/change to change all the numbers in the index to something like “1:45” (for issue 1, page 45); 3) combining the indexes; 4) using sort to sort them properly.

David BlatnerKeymasterOh, one other thing popped into mind: After importing, did you look at it in Story Editor (Edit > Edit in Story Editor)? I wonder if the text really is there but is invisible on the document page for some reason.
David BlatnerKeymasterThat does sound really weird. I haven’t seen that before. While RTF should work, I would try doing a save as and importing it as .docx if possible. Something must be really odd with that file.

David BlatnerKeymasterYou should leave it set to Facing Pages, leave the bleed on top, bottom, and outside, and just export a PDF (set to Pages not Spreads, of course). Basically, probably the same as you did for Createspace. If the Lulu book will have facing pages (left and right), then you should not change your ID document to single sided. That is a mistake, no matter what printer you use. (Unless you are printing for spiral bound binding, or on single pages that will go into a binder, or something like that.)

David BlatnerKeymasterNicolas, if only you hadn’t let go of the sorcerer’s stone… that stone allowed you to apply more than one character style to text. But without that, it’s impossible.
I’m kidding about the stone, of course. But the truth is that InDesign only lets you apply one character style. If you change the character style, it removes the old one and applies the new one.
The only solutions would be to make a third character style that applies both superscript and color… or you could first select the text, choose Break Link to Style from the character style panel menu (at which point the first style would be converted to local/manual override formatting), and then apply another one.

David BlatnerKeymasterMy guess is that the text is being converted to outlines. That is really strange. Have you tried different PDF/X export presets, such as PDF/X4 export?
