MemberThe default in InDesign is one page on top (so, as you said, Page 1 is on the right). If you have 2 pages on top (a spread), then Page 1 would be on the left.
Don’t fight InDesign on this! They know what they are doing!
Your main concern should be that each chapter has an even number of pages, so that the first page of each chapter lands on an odd number. You may need to add a blank page before your graphic page to achieve this.

MemberUse the first page as the first page of your chapter.
Put the graphic on the last page of each chapter.
Make sure each chapter has an even number of pages.
Example:
Chapter 1: 1 – 2 -3-4 (graphic)

MemberIf the InDesign file is print ready, you don’t need a PDF to print, just print from InDesign.
Otherwise, print the PDF from Acrobat. If bleeds and crop marks are needed, make sure they are turned on.

MemberIt’s hard to say without seeing your PDF, but some suggestions:
1. Make sure there are no spot colors in your EPS and InDesign file (If you have raster effects such as transparency and drop shadows, spot colors will ruin everything)
2. Make sure your EPS files are CMYK (File->Document Color Mode)
3. Export to PDF x/1a
4. Check separations in PDFRich black is not universal, I assume your printer gave you those builds? If not, they may have changed them on one and not the other.

Odd pages on the right are normal for English language. (When you open a book, Page 1 is on the right)
Right click on the page you want to number Page 1, Numbering and Section Options, and choose Start Page Numbering at 1. (you should name previous pages something besides numbers)
Check your Preferences -> General – you may want Absolute Numbering or Section Numbering.

MemberSounds like a Text Wrap issue. Try these things:
Select cover image ->Window->Text Wrap (disable it or adjust)
Check Layer Options ->Suppress Text Wrap When Layer is Hidden
Select text frames->Object->Text Frame Options->Ignore Text Wrap

MemberI am by no means an expert on color management, but the print shop I work at just hired an expert. He set things up so every CMYK job to every device is converted to a GRACOL profile.
We print on an HP Indigo, which is also our proofer for our offset press.
I gave up on trying to match screen to press; there are too many variables, so I just adjust colors/saturation to the printed proofs.

MemberIt’s nothing you did!
For some reason, a lot of vector artwork gets set to Overprint by default when outlining fonts, and it gets set by accident in Illustrator. Most people don’t notice if they never have their work printed.
I don’t think you change Overprint to several items in InDesign, but you can copy and fix all in Illustrator. The main thing in InDesign is to check Attributes with both the Selection and the Direct Selection tool.

MemberIt may be an Overprint issue (common with vector art).
Try this:
Select dingbat with Direct Selection tool (open arrow).
Window->Output-AttributesIf Overprint fill is checked, uncheck.

I think you were very close.
Here’s what I did with your file.
1. Select Master Page text frame (Edit->Select All with Type Tool, Edit->Cut)
2. File->Document Setup->1 page (deletes the other 99)
3. Pages->Insert 1 page from Master
4. With Selection Tool, click on red + on page 1 text box
5. On Page 2, Shift and Click over text box. (Shift tells your text to flow to other pages)That should add the rest of your page numbers.
