I’ve got three columns in one text box — One column headline and subheadline … one photograph and a photo caption — all 15.3 leading, Top, baseline … so why does the last line in column one end one line HIGHER than column two and three?

Try using only cardinal numbers for leading and have all your text link to baseline.
Your photograph should sit within the baseline and the text wrap should be your baseline height.
If your caption is smaller, then you can halve your baseline grid, making sure that your paragraph spacing is set accordingly.

However, attach a screenshot and/or your INDD file for clarity.
