Unable to create new hyperlink in index file (CS5)

      I am trying to create hyperlinks from an index to anchored text within other documents within a book. I am able to create the desination hyperlink, but when I go to the index document and try to create a new hyperlink, the option is grayed out. I have no idea what I'm doing wrong, I've been trying to get this figured out all afternoon. Any help or suggestions? Thanks.
