      I have set up 2 style sheets and they only work some of the time. Any ideas what could be happening?

      The last para heading “CULTURE IN RECOVERY” needs a new paragraph (return) after it – by the look of the image, it only has a “new line”.
      Is that the problem?

      Chris is correct you are using soft returns between many of your headers and body text.
      It looks like you have been doing some manual formatting as the first 4 paragraphs have soft returns.
      Change them all to normal paragraph returns then apply the Paragraph Styles.
      If the text is as you show make the Header style to be followed by Body text and Body text followed by Header, select all the text and apply Header text and then next style and all the text will format correctly as shown.

      You can use Find and Replace to change the soft returns.

      If I change the soft return to a paragraph return the leading is too big no matter how creative I get. Is there some thing I am missing that you can think of?

      With the two paragraph styles check that they have the correct leading and Space Before (on the headings) or Space After (body text) that you need to get the text space correctly.
      Have you removed all Character Styles from the text?
      All the formatting needs done with Paragraph Styles.
      Are you using align to baseline grid at all? This can easily cause a issue if not used correctly.

      I’m getting close but I’m not there. Thanks to your advice about the paragraph styles I am happy to say.

      Also,
      I know it’s probably not advisable but I had a huge problem with the multiple soft and hard returns. Any alternatives given the situation?

      Your imgur link is not working so we can’t see your example. Post an example of your issues and we can see if we can help.
      You can make girds for each text frame. Use TEXT FRAME OPTIONS and then the BASELINE OPTIONS tab and click USE CUSTOM BASELINE GRID. This will allow you to make a grid for each text box.
      For returns it is best to use the type you actually need for your purpose, Standard Carriage Return, Soft Return and Discretionary Line Break etc.

      I’m wrestling with the leading and have attached a screen shot to show the problem. I have made an arrow next to the treatment I am after. https://imgur.com/a/3QXmuQR

      I’ve tried matching leading, changing the font and I can’t get it to do what I’m looking for. The only thing that works is adding a soft return but I can’t get it to work in a style sheet.

      You have one para style with 10pt leading and one with 11pt leading and for each the font size is the same.
      The one with 11pt leading also uses ALIGN TO BASELINE GRID when you use this the leading etc will be overridden by the grid. Turn off the align to baseline grid or make all of your styles use a multiple of the grid spacing for leading, space before and space after. If your gird is say 10pt and then you use 11pt leading the text will actually end up having 20pt leading, twice the grid, or if you use 8pt leading it will end up at 10pt, the grid spacing.
