problems generating an index

      I am selecting entries, and the system is generating an index marker where and as it should, but when I generate the index the following things occur:

      1. the new entries aren't showing up

      2. not all references for any one entry are showing up

      3. the number of entries that show up changes each time; overall, fewer entries show up each time.

      A review of the text reveals that all markers are still in the text.

      I have no answer, but am dealing with a similar problem. Did you ever figure out a fix?
