Page numbers in TOC are incorrect (offset)

      Problem: In a generated TOC, all the page numbers are incorrect, but the numbers are not random; rather, they are all off by the same fixed number. In my case, they are all off by -9: page 16 is listed as page 7, page 19 is page 10, and so forth. The page numbers in each document are correct, and as far as I can tell, the book and document numbering properties are all consistent. Also when I generate a PDF, the hyperlinks all work (for example, the TOC entry for a heading on page 7 goes to the correct heading on page 16).

      The formatting is pretty straightforward; there are no L/R page formats, and page numbers are simple (just the page number; no prefixes, chapter numbers, etc.). The page count is just over 220 pages distributed among the following documents: Cover, TOC, Preface, and three chapters.

      Unfortunately, can’t forward actual samples as the book contains proprietary information.

      My guess is that you have 9 front pages which are numbered with letters or roman numerals and then numbering starts again. In this case, page 1 would be physical page 10, page 11 would be page 2 and so on.
