      This an entry in my index:
      Braddock’s Defeat 253,387
      Braddock’s Defeat 335

      I have reindexed many times but have not solved this. I am using Indesign CC 2015. I have also checked hidden characters.

      Have you gone to that reference in the book and looked for the telltale index marker hidden character?

      I did that but wasn’t sure what I should be looking for.

      Go to the original reference in the text, select nearby words, and open the Story editor (Command/Ctrl-Y). The same text will be selected but the index marker will be much easier to spot, it looks like a little guy waving 2 semaphore flags. Delete it, then regenerate the index, see if that fixed it.

      I did that but no luck. One thing I have noticed follows:
      Original entry in my index: Chadd’s Ford 514
      Chadds Ford 432 I added an Apostrophe
      Chad’s Ford 476 I added a d

      Result: Chadd’s Ford 476, 514
      Chadd’s Ford 432
