Lists of figures and tables generate out of order in front matter

      When I generate a list of figures or tables, the order is often (but not always) out of order if 2 or more figures/tables appear on the same page of text. The lists generate fine, except for that problem. I've wracked my brain but just can't come up with a solution. Any help would sure be appreciated.
