Learning guidance for form and questionnaire creation

      Hello All,

      I have been given the task of helping attorneys create inviting questionnaires for individual areas of practice (e.g., bankruptcy, consumer protection, etc.).

      I have all the information which should be on the questionnaires/forms, but it seems that attorneys are a little boring with the presentation/attractiveness of the questionnaires/forms.

      (I have seen attorneys' clients literally cringe when they saw the very long and BORING 30+ page questionnaires/forms.)

      Anyone know of a good book, tutorial, white paper, or other method of learning how to design attractive questionnaires/forms?
