    My problem is solved! I discovered that the images were in grayscale mode and InDesign converts them to RGB when I exported them to EPUB. Thats the reason why InDesign could not use the existing image. After convertering the images to RGB (with Irfanview) all worked fine:-)

    The solution was just in front of my eyes. It was the size of the original images that varied.

    Thanks for the detailed comments on my question. So far it has been iBooks that have had the most support for fixed layout, as far as know. Nobody is probably going to read photo books or children’s books on a mobile small screen. I will probably continue to work with Epub 2 and 3 for variable layout and PDF format for fixed layout.
