Going off Dieter’s suggestion, you could place these headers on a hidden/non-printing layer and use them to build the TOC.
If the instances of she/he are not set up like this with the slash, and you mean either “she” or “he”

This is definitely not my wheelhouse, but I have a suggestion for you to look into further. See about placing 1 large text box on each page, and anchoring the image/captions within the text box. The textbox is then threaded over the remaining pages. This way, if you removed image #2, all following images will reflow. At least in my mind this works. Good luck!

So are you looking to auto-fit the text to the width of the box? Like if you type 1 extra letter, the whole line goes down by 1 or 2 pts?
2 suggestions for you:
First, I’ve been having trouble getting my FindChangeByList queries to work if I haven’t included the line {includeLockedStoriesForFind:false, includeLockedLayersForFind:false, includeHiddenLayers:false, includeMasterPages:false, includeFootnotes:true, kanaSensitive:true, widthSensitive:true, searchBackwards:false} at the end of each query.
Second, make sure you have a tab character in between glyph and {findWhat, } and {changeTo.
