David BlatnerKeymasterIt's very hard to know what could be wrong here, but I wonder if perhaps some kind of minor document corruption has snuck in. You might try exporting the document as INX or IDML, then opening that file, then exporting to epub. Does that work?

David BlatnerKeymasterThat's exactly why Adobe added Line Styles in CS4… no way to really do it in CS3 well, unfortunately. But Line Styles makes it really easy!

David BlatnerKeymasterDoublespace means “two times” so the leading would need to be twice the font size. Howevever, InDesign only allows percentage leading as part of “Auto” leading, which you can define in the Justification dialog box.
