After some tests I found a way to avoid to create a TOC in a separate file (and add the file to the indb, repagination, etc, etc). I had the same issue using Indesign CC 2014, in a indb book with 19 chapters and a perfectly done TOC (made inside a prologue part).
I moved the main source file mark (the right icon that appear only in one file of the book) to the file that have the TOC inside (where I created the TOC style), after that I saved again the book and surprise! my TOC NAV style was there!!! I exported again my epub file and the index looks good! (just I desired). I hope this appear in the future in the Adobe help material, I checked some videos and tutorials but not there at all.
