I thought of doing that. But when a section heading is pushed to the top of the page/column then it has an unnecessary space at the top. Any thoughts on how I could eliminate the space?

I ended up using a custom cross-reference to add “see” and “see also”, and then creating an italic style for it and applying it in the Index dialog box. It’s ugly, but it works.
