The effective resolution is 300dpi. It turned out to be something in the text box over the image. I made a new text box (duplicating it didn’t resolve the problem), reset the text and this fixed it. The publisher probably used a previous cover by the same author to create the new one. It’s possible the problem was created by converting an older version document, although we’ll probably never know…
