The quick version is that you will need at least two different styles (one for each 1.x, which restarts the numbering as part of the style definition.)

re: graphs. This is a perpetual problem. I don't know where you read that you can copy and paste graphs, but I hope it wasn't on this site. Copy and paste is not a good idea. Save as PDF/EPS and open in Illustrator for fixup.
