It says very clearly, in an example (in the “Interpretation” section):

“For example, a company hosts a free webinar about its products for the general public. After the webinar, it sends a Reader Extended product survey PDF form to thousands of attendees. An Acrobat license allows the company to extract information from up to, but not more than, 500 surveys, regardless of the number of completed surveys the company receives.”

So, you can send out unlimited PDFS, but you may only collect information (for each survey) from up to 500 people. Each survey can be the same or different 500 people.
